Frequently Asked Questions

Frequently Asked Questions About Physical Therapy Park City

Do you take insurance?

I am an out-of-network physical therapy practice and do not network directly with any private insurance companies or Medicare/Medicaid. This allows me to provide one-on-one, highly individualized treatment based on what you need and what your goals are without your care being dictated by insurance companies. This allows us to take a proactive approach to treatment and healing, working to solve issues as they arise and address the body and healing in a preventative way with the goal of long term maintenance of good movement and function.

Can I get reimbursed for treatment from my insurance company?

Depending on your benefits, some insurance companies will reimburse at an out-of-network rate or count the cost towards your deductible (this is not applicable for Medicare). I can help guide you through what questions to ask your insurance provider to determine what your benefits are and can provide you with necessary documentation to submit for reimbursement upon request.

Do you accept HSA/FSA as forms of payment?

I do accept HSA and FSA.

What are your rates?

My rate is $175/hour. Visits are all one-on-one in a private setting where we have the dedicated time to develop a highly personalized treatment plan based on your condition and goals for treatment. Typical visits are 60 minutes with the option for 90 minutes ($250).

I believe in the right to full transparency and understanding of healthcare costs up front. There are no surprise or hidden costs associated with treatment.

What are accepted forms of payment?

I accept all major credit cards, cash, check, HSA and FSA.

Do I need a prescription or referral from a physician?

Physical therapy is recognized as a direct access healthcare service in the state of Utah, meaning that you do not need a prescription or referral from a physician.
